Sibo - small intestinal misalignment
A contribution by alternative practitioner Edda Federspiel
Do the following statements seem familiar to you?
- Do you suffer from a variety of symptoms in the gastrointestinal tract, histamine intolerance, inexplicable pain in muscles and joints, numerous food intolerance and allergies, neurological disorders maybe even depression?
- You don't tolerate pre- and probiotics?
- Do you often react negatively to plant substances and nutritional supplements?
- No doctor can help you?
- Already made all investigations?
- Hasn't the intestinal structure either?
Then it may be that you suffer from a small intestinal misalignment.
👉🏼 50% of all Sibo patients also suffer from histamine intolerance!
What is Sibo?
A deflection of the small intestine can occur from immigrant bacteria from the large intestine or via bacteria from the upper digestive tract, e.g. Chron. Calconal infections, pine and tootheds or dead teeth, which are constantly flushing bacterial toxins into the gastrointestinal tract via saliva. If stomach acid is still missing to kill the bacteria, it is easy to populate the small intestine, the so-called overgroth-syndrome (or Sibo).
The bacteria in the large intestine (approx. 1 trillion per ml) split fiber, among other things, while bacteria in the small intestine (approx. 1000 per ml) are primarily responsible for the nutrient absorption.
In the normally bacterial small intestine there is a abnormally high number of bacteria with a shifting of grampositive to gram -negative Bacteria. These immigrated bacteria produce various gases in fermentation of starch and fiber, namely hydrogen, methane or hydrogen sulfide. The resulting symptoms can be very different, depending on the type of the moved bacteria. The symptoms are not only limited to the gastrointestinal tract, but also ranges from respiratory problems, metabolic disorders, nutrient deficits to neurological or immunological disorders! Due to the many non -specific physical complaints, Sibo often remains undetected for years. Unfortunately, there are not enough therapists who are familiar with the problem.
Effects of Sibo
- Gastrointestinal tract: Diarrhea, nausea, flatulence / bloated stomach, reflux, belt, fullness of fullness
- Metabolic & immunological effects: Leaky good, multiple food intolerances, histamine intolerance, allergies, asthma, tiredness/fatigue, joint pain, weight loss, etc.
- Neurological symptoms: Forgetfulness, brain fog, headache, anxiety, panic attacks, depression
- Skin: Rashes, pimples, hives, eczema
- Nutrient deficits: Vitamin B 12, iron, vitamins ADEK (= fat -soluble vitamins), folic acid, magnesium, calcium etc.
👉🏼 It is estimated that around 9.5 million people in Germany are affected by Sibo/IMO, with approximately 80% of the irritable bowel patients having a small intestinal misalignment.
There is also a particularly frequent occurrence: diabetes, autoimmune diseases, chron. Infections such as Lyme disease and liver diseases.

3 different types of Sibo and their symptoms in the digestive tract
hydrogen = small intestinal bacterial overgrowth (Sibo)
- Diarrhea, or alternating diarrhea and constipation
- Abdominal cramps
- Fibromyalgia
methane = intestinal methanobacter overgroth (IMO)
- constipation
- nausea
- Bump off
- Flatulence
Hydrogen sulfide
- Flawy -smelling flatulence
- Diarrhea
- Bladder pain
- Joint pain
Diagnosis
Sibo can only be diagnosed using a breathing guest test!
Bacterial fermentation using lactulose/ glucose and recently also fructose is evaluated.
Hydrogen + methangas are measured. Hydrogen sulfide cannot currently be measured by us, possible only in America.
From the laboratory you get a breath test with a precise description that is carried out over 3 hours. The exhaled air is measured every 20 minutes.
To prepare:
A special diet and a 12 -hour fasting period must be observed before the test
4 weeks before:
No colonoscopy, no antibiotic intake
1 week before:
No gastric acid blockers, no pro and prebiotics, no fermented foods
1 day before:
No enzymes, laxatives, nutritional supplements, magnesium + vitamins

Consequences of Sibo
- Leaky good (permeable intestine),
- Histamine intolerance,
- Vitamin and mineral defects,
- Motility disorders of the intestine (limited intestinal movements),
- Gallen and liver disorders due to poor absorption of fat-soluble vitamins and loss of bile salts
Causes of Sibo
Food poisoning is often triggered by Sibo or the years of taking acid inhibitors!
- Motility disorders (Movement disorders) of the intestine: for irritable bowel, after traumatic brain trauma, in hypothyroidism, chronic inflammation, diabetes, mold pollution, scleroderma, weak vagus nerve
- Digestive disorders: Microbioma disorders, chronic stress, a lack of stomach acid, a lack of bile acid, enzyme deficiency, disorders of the defense of mucous membrane (secretion IGA deficiency)
- Structural problems = Drainability affects by: dysfunction of the ileozäcal flap (= construction flap = closure flap between thin and large intestine), Ehlers-danos syndrome (hypermobility of the joints), endometriosis, scar formation and adhesive after abdominal operations
- Medication: Opiates, sedatives, gastric acid inhibitors, antidepressants, thyroid medication, antispasmodic medication
- Stress, toxins, parasites, enzyme deficiency, autoimmune diseases, thyroid diseases, intestinal inflammation, hormone disorders And other diseases can contribute to the development of Sibo
A good one is very important Motility The intestine, because it is a prerequisite that bacteria, cell debris, food residues and waste are transported from the small intestine to the large intestine. This only happens after 4-5 hours of food or during sleep.
Every 90 minutes there is a natural cleaning wave in the small intestine, which is triggered by the enteric nervous system and the peptide hormone Motelin. If this does not happen enough (e.g. through constant food), too many bacteria remain in the small intestine and lead to overgrowth. The resulting LPS germs play a crucial role in this.
High amounts of LPS can be found in people with higher proportions of prototobacteria and bacteroidet in the thick and small intestine. The formation or increase of the LPS germs depends heavily on the diet and taking medication (e.g. gastric acid inhibitors).
LPS germs are the degradation products of gram -negative bacteria. The resulting toxins can cause inflammatory processes in the intestine, and they also reduce intestinal peristalsis and inhibit the cleaning wave!
The resulting stress reaction suppresses the dorsal vagus nerve (Ast of the vagus nerve running over the back), which inhibits both the stomach acid and the nutrient absorption in the intestine. There is also a reduction in detoxification and anti -inflammatory inhibition. In contrast to the ventral vagus (intestinal brain brain axis), these reactions are done, which supports and promotes digestion!

How can you test yourself if you may have suffered from Sibo?
Fillers, strengths and fodmap-rich foods are fermented by bacteria at Sibo and lead to corresponding symptoms! These include: various fruits, vegetables, nuts, grain, lactose, sugar and legumes.
In order to test whether a Sibo could be available to you, pass temporarily:
- Onions, garlic, celery
- Apple, peach, pear, watermelon, possibly also banana
- Whole grain products, rye
- Legumes
- Dairy products with lactose
- Maiss syrup, sweetener,
- Nuts & seeds
- Raw food, green salad
- various prebiotics such as inulin, zichorie root, fos, gos, psyllium husks, arabinogalactans
In the case of Sibo, they serve the wrong bacteria as food !!! And even lead to an increase and thus to worsen the symptoms!
Keep a distance of 4 to 6 hours between meals, do without snacks and calorie drinks! Take time for food, chew every bit very well.
If you react to the change of food with an improvement in your symptoms, there is probably a Sibo.
Other ways to relieve the symptoms:
👉🏼 stress inhibits the natural intestinal movement (motility) that is necessary to transport the food through the entire digestive tract. If this does not happen enough, constipation (constipation) occurs with long -reaching consequences.
👉🏼 Sleep hygiene Support: Ashwagandha (has the opposite effect in some people!), L-tryptophan, magnesium, B vitamins, melatonin
👉🏼 Castor oil wrap: It is best to drink with a plastic film or towel in the evening before going to bed, soak with castor oil, put on the lower abdomen, fix with a large binding and put on a hot water bottle. Let it work in for at least 1 hour and rest! Helps to improve the intestinal movement and the cleaning wave.
👉🏼 Abdominal massage According to Dr. Nirala Jacobi
👉🏼 Breathing exercises: For example, mutual nasal breathing or abdominal breathing
What to do if the Sibo test is positive?
Therapy options
- Elementary nutrition according to Dr. Mark Pimentel, USA or low-Fodmap diet for approx. 2-3 weeks
- Special antibiotic treatment must be repeated in persistent cases (Beware of herxheim reaction possible!)
- Depending on the Sibo art: Rifaximin, Neomycin, Metronidazole - Medical Ordinance
- Vegetable antimicrobial funds for approx. 4-6 weeks, possibly apply repeated depending on the Sibo art: Berberine, oregano oil, neem, kno blue extract, various essential oils, bismuth or others Important: Use individually selected funds, depending on the tolerance! Best with therapeutic accompaniment! ((Beware of herxheim reaction possible!)
- Improvement of intestinal engineering with medication: ginger preparations, artichoke preparations, Iberogast drops and others, depending on the compression recessation (see above)
- Leaky good therapy (with a positive stool test)
- Prebiotics only after Sibo therapy. No inulin and psyllium husk! Otherwise not only the good bacteria are fed, but also those we want to fight. Exceptions are: Gos (GalactoOligosaccharide), possibly Acacia fibers (Check compatibility!)
- Use of special probiotics that have to be selected individually
- Treat possible interference fields that prevent recovery: Candida, leaky good, false probiotics, persistent stress
- After treatments for a long time
Treatment is very individual and can be very lengthy, but a healing is possible if you stick with endurance and consistency. Without an expert therapist who accompanies the various treatments, chooses the right pre- and probiotics and medication, it can be very difficult and you may lose a lot of time with useless or false and costly therapies.
If Herxheimer reactions occur, it is important to have a competent therapist who knows how to meet this problem and what to do. A Herxheimer reaction can occur if bacteria die through antibiotic therapy or herbal therapy. Their decay products can lead to severe symptoms, such as chills, headaches, nausea, vomiting, rashes up to shock. In less severe cases, e.g. activated carbon, humin or fulvic acid, tonerde or other toxin binders help to bruise these reactions.
I would like to thank Sabine Hofele, whose online seminars were allowed to learn a lot. It is on the road worldwide to be constantly up to date with Sibo, mast cell activation and Borreliosis diseases.
