What is hives?

Nettle addiction is one of the most common skin diseases and affects about 20% of people at least once in their lives. It is often associated with allergic reactions, but in many cases it is not due to allergies. Typical signs are the formation of wheels that are reminiscent of mosquito bites. Symptoms

The main symptom of hives are the whispered wheels and the strong itching. Other symptoms can be:

  • Redness

Causes

The causes of hives are diverse and often not clear. Here are some of the most common triggers:

  • Food allergies, pollen, dust mites or animal hair can trigger an allergic reaction that leads to hives; Food allergens are nuts, eggs, fish, shellfish and dairy products.
  • Viral infections such as colds or bacterial infections can cause hives; In some cases, the hives only occur after the infection subsided
  • Medicines, especially antibiotics, pain relievers (e.g. aspirin, ibuprofen) and blood pressure lower
  • Emotional stress can trigger hives in many people - stress affects the immune system and can stimulate the mast cells to release histamine
  • Stimuli such as heat, cold, sunlight, water, pressure or friction
  • Hormonal fluctuations, especially in women, for example during menstruation or pregnancy

In many cases there is no clear trigger - this form of hives is called idiopathic urticaria.

diagnosis

The diagnosis of hives is primarily due to a physical examination and the survey on the symptoms and possible trigger. Among other things, the doctor will ask you whether you have taken new medication, which food you have eaten or whether you are under stress. In some cases, special allergy tests, blood tests or provocation tests are also necessary to identify possible causes.

With chronic hives, it is particularly important to find the exact triggers, since this form is often more persistent. Leading a symptom diary can be very helpful here. In it you can note when the symptoms appear, what you ate, whether you were under stress or whether other factors such as weather changes could have played a role.

🔹Urtikaria or anaphylactic shock?
Not every wheal is a sole sign of a urticaria. This means that squaddling can also occur as a symptom of an allergic shock (anaphylaxis)- but there are also other mast cell and histamine-mediated complaints such as breathing problems and circulatory disorders. It is crucial to differentiate between an acute urticaria (which only shows whispering and/or angio edema) and anaphylaxis, since the latter represents a more comprehensive, often life -threatening state!

🔹Inducable urticaria and provocation tests
In addition to the spontaneous chronic urticaria, there are inducable shapes (e.g. cold, heat, water or light aprils), in which specific physical stimuli trigger quaddling.
A provocation test is often carried out when suspected an inducable urticaria (e.g. cold metropolitanies). In the past, an ice cube was enough, today devices such as the temp test are used that irritate the skin with different temperatures. In this way, it can be determined from which temperature whereas - which not only indicates the severity of the disease, but also helps with therapy monitoring.

The role of the mast cells and histamine

An important point in hives is the role of the mast cells. These cells belong to your immune system and sit directly under the skin and in the mucous membranes. Mast cells contain various messenger substances, one of the best known is histamine. If the mast cells are activated, they release histamine, among other things, which leads to the typical nessel addiction symptoms. Histamine means that the blood vessels become more permeable. As a result, liquid occurs from the vessels into the surrounding fabric, which leads to squaddling, swelling and the intensive itching. This reaction can be triggered by various stimuli, including allergens, physical stimuli (such as cold, pressure or heat) or stress.

3. Immunosuppressant: In very severe, chronic cases that do not respond to other treatments, immunosuppressants such as ciclosporin can be used to suppress the immune system.

4. Biologics: A relatively new form of therapy are biologicals such as omalizumab. These antibodies are specifically against the inflammatory processes in the body and are particularly effective in chronic hives.

5. Avoiding the trigger: If the hives are due to certain triggers such as food or medication, contact with them should of course be avoided. In many cases, physical triggers such as cold or pressure can also be avoided or at least minimized.
